Ceresti’s digital caregiver empowerment program enrolls and upskills the family caregivers (e.g., spouses, adult children) of frail elderly persons living with dementia and engages them in monitoring the health of their loved ones via remote risk assessments. Enrolled caregivers receive a dedicated cell-enabled caregiver tablet that provides access to a comprehensive and personalized program of education, support, proactive remote coaching and remote assessments. Remote assessment enable the early detection of changes in the condition of the person living with dementia, and timely access to primary care, obviating the need for emergency room visits or hospitalizations. Caregivers typically engage in their programs for 40 minutes/week over a period of 7 months.
