Qure4u Launches Shared Care Plans and Chronic Care Management Platform

BRADENTON, Fla. - March 14, 2016 - PRLog -- Qure4u, Inc. announced today their cloud-based, patient-centric, health management platform will now include Shared Care Plans and a comprehensive suite of tools for Chronic Care Management.

According to the Centers for Disease Control, eighty-six percent (86%) of all healthcare spending in the U.S. is for people with one or more chronic medical conditions. About half of all adults in the U.S. (117 million people) have one or more chronic conditions.

As part of the Affordable Care Act, the Centers for Medicare and Medicaid Services has introduced incentives for physicians and healthcare delivery systems to improve the coordination of care for persons with chronic conditions. Private payers are also introducing similar incentives.

Secure messaging, texting, video calls, test results, referrals and appointments can all be conducted within the HIPAA compliant platform.
